What is One Medical Passport?

Completing a One Medical Passport medical history online is easy. For most patients, filling out the entire questionnaire takes less than 15 minutes. Please fill out the questionnaire accurately, and be assured that all of your information is kept confidential and will be thoroughly reviewed by your medical team.

One Medical Passport is a website that allows you to enter your information at any time from anywhere. You can also print out a copy of your medical history after you create it online and keep it with you or with your other health care documents, as well as have access to it online anytime you need it or want to update it.

Information Security: One Medical Passport uses the highest grade of Internet security available: 128 bit encryption with SSL (secure socket layer) technology. Your information is stored on servers at a secure web hosting site and will only be released to your medical team at Stratham Ambulatory Surgery Center. Your information is never released to anyone other than your medical team.

What are Ambulatory Surgery Centers?

Ambulatory Surgery Centers are centers where surgical or other medical procedures can be safely performed without requiring the patient to stay in the hospital overnight.

These centers are also sometimes referred to as surgicenters, same day surgery centers or outpatient surgery centers.

When will I be discharged from the center after my surgery?

Studies show people feel best recovering in the comfort of their home environment. Prior to being discharged to home, all of our patients are awake, alert and have adequate pain control. Furthermore, any and all questions/concerns our patients have will be answered by our staff before you go home.

When I return home from my surgery, will I be able to resume my usual daily activities, such as caring for my small children?

After you return home, it is very important that you follow the home care instructions provided by your surgeon and the center. If you’ve received anesthesia, you need to arrange for someone to stay with you for 24 hours after you return home. In addition, you may want to have assistance with caring for your children and/or large pets. Check with your surgeon prior to discharge regarding your schedule to resume normal activity.

Whom do I call if I have questions about my postoperative instructions?

Please feel free to call your surgeon or the center for any questions you may have at any time. After your surgery, your surgeon will speak with your family or friend to provide an update on your status. Your surgeon may speak with you after your surgery; however, due to the effects of anesthesia you may not recall the conversation. This is why your doctor speaks with your family/friend as well.

Should I take my routine medications and supplements?

Please contact your surgeon to discuss which medications you should or should not be taking.

Is there any sort of entertainment accommodations for me to pass the time while I await my surgery?

Stratham Ambulatory Surgery Center has available free Wifi, telephone, TV/DVD, current magazines and publications. Feel free to bring a DVD to watch.

What if an emergency arises and I am not able to make my appointment?

Emergencies can happen. As soon as you know you are unable to make your appointment, contact both SASC and your surgeon right away.
